End notes are listed on this web site together with the web links to the trials where known to be online in summary or in part. Much of the research was done through web searches. Without the web this book would not have been possible.

Footnotes will be added to the site at some future point.

Finding references from end notes from can be time consuming. I find it very helpful where papers carry links to the references they make, which is why I posted the notes and links.
